Erik S. Crook, M.D.
Dr. Crook joined The Columbia Medical Group in July 2008 after graduating from the internal medicine residency program at the Medical University of South Carolina. Before attending medical school at MUSC, he graduated from The University of South Carolina Honors College with a B.S. in biology. Dr. Crook is certified by the American Board of Internal Medicine. He has hospital privileges at Palmetto Health Baptist and Providence Hospital.

Edgar G. DesChamps, M.D.
Dr. DesChamps is originally from Bishopville, SC but has lived in Columbia since joining The Columbia Medical Group in 2003. Dr. DesChamps has been in private practice since 1985 and is certified by the American Board of Internal Medicine. He attended the University of South Carolina obtaining a B.S. in Biology. He continued his education at the University School of Medicine receiving his M.D. and completed a residency in internal medicine. Dr. DesChamps has been the State Medical Director for DHEC’s Division of Emergency Medical Services since 1995. He also practices emergency medicine in one of Columbia’s hospitals. Dr. DesChamps has hospital privileges at Palmetto Health Baptist and Providence Hospital.
Dr. DesChamps is an NCQA Recognized Physician.

Rosa Jimenez, M.D.
Dr. Jimenez has more than 20 years of medical experience as a practicing Internal Medicine Physician. She is a founding partner of The Columbia Medical Group. Dr. Jimenez received a B.S. from the University of Puerto Rico where she also obtained her Doctor of Medicine. She completed her residency at Eisenhower Army Medical Center, Fort Gordon in Augusta, GA. Dr. Jimenez is certified by the American Board of Internal Medicine.
Dr. Jimenez is an NCQA Recognized Physician.

Ruben L. Mayer, M.D.
Dr. Mayer has been practicing medicine since 1988 and is a founding partner of The Columbia Medical Group. He received a B.S. from the University of South Carolina and obtained his M.D. from the USC School of Medicine. Dr. Mayer completed his residency training at the University of Virginia Hospital in Charlottesville, VA before returning to South Carolina to practice medicine.  He is certified by the American Board of Internal Medicine. Dr. Mayer has hospital privileges at Palmetto Health Baptist and Providence Hospital.
Dr. Mayer is an NCQA Recognized Physician.

Kim Meadows, M.D.
Dr. Meadows has been practicing medicine for more than 20 years and is a founding partner of The Columbia Medical Group. She received a B.S. from the University of South Carolina and obtained a Doctor of Medicine from the USC School of Medicine. She completed her residency training in internal medicine at Portsmouth Naval Hospital in Portsmouth, Va. Dr. Meadows is certified by the American Board of Internal Medicine.
Dr. Meadows is an NCQA Recognized Physician.

Stacie Moore-Bowens, M.D.
Dr. Moore-Bowens joined The Columbia Medical Group in 2004 and has been practicing medicine in Columbia since 1996. She received a B.S. in Chemistry from the College of Charleston in Charleston, SC. Dr. Moore-Bowens obtained a Doctor of Medicine from the USC School of Medicine where she also completed the internal medicine residency program. She is certified by the American Board of Internal Medicine. Dr. Moore-Bowens has hospital privileges at Palmetto Health Baptist and Providence Hospital.
Dr. Moore-Bowens is an NCQA Recognized Physician.

Bradley W. Word, M.D.
Dr. Word has been practicing medicine since coming to Columbia in 1995. He obtained a B.S. from Pittsburg State University in Pittsburg, KS.  Dr. Word received his M.D. from Vanderbilt in Nashville, TN where he also completed a residency in internal medicine. He is a founding partner of The Columbia Medical Group and is certified by the American Board of Internal Medicine. Dr. Word has hospital privileges at Palmetto Health Baptist and Providence Hospital.
Dr. Word is an NCQA Recognized Physician.

David H. Hammett, M.D.
Dr. Hammett will join The Columbia Medical Group on May 12, 2010 as the group’s first Neurologist. He completed a fellowship in Vascular Neurology as well as his residency at Vanderbilt University. He graduated from medical school at Bowman Gray School of Medicine at Wake Forest University. Dr. Hammett specializes in vascular neurology and stroke, memory disorders and dementia, headache disorders - including procedure-based treatments, spasticity and movement-based disorders – including procedure based treatments, and general neurology. Dr. Hammett is accepting new patients.
